Medical History for Dental Offices - PALM reference is limited to
Cardiac concerns. The second is a full reference. This one covers
ALL organ system, Cardiac, Pulmonary, Gastrointestinal, etc. |

You get a 7 day trial period with full functionality.
You can try it before buying. After 7 days, the application will not
run. In order to use the application after 7 days, you will need to
enter an unlock code. Once we receive payment, we will contact you
via e-mail for some information that will allow us to generate your
individual "unlock" code.

These PALM applications require a runtime file called
"Booster". If you do NOT have "Booster" installed on your PALM, select
2 or 4 below. The applications were originally written to work with
PALM OS 3 and 4. If you already have "Booster" installed,
then choose 1 or 3 below.
If you are using a PALM OS 5 handheld, you must first determine if
a "Booster" is available to you. There are separate versions
of "Booster" for most PALM OS 5 handhelds. You will have
to check the
AppForge site to determine if your handheld has a "Booster"
available. If there is a "Booster" available, these applications
should work. We are taking the word of AppForge on this (it has not
been thoroughly tested on PALM OS 5). |
