Medical History for Dental Offices - PALM Demonstration

In the Medical History form in the PALM application, you are presented a series of Medical conditions. If the patient gives an affirmative answer to a condition, just scroll to the appropriate condition, highlight the condition by touching the line on the PALM screen with the stylus, and then touch the "Warn" button.

As an alternative, as you get more comfortable with the application, you can enter the number of the condition or the first 3 letters of the condition in the "enter the Name or #" text field in the lower left of the screen. For example, if you want to jump to the highlighted line, you could enter "7", "irr" (for IRRegular heart beat), or "arr" (for ARRythmia) and the program will automatically go to line 7 and highlight it. To see the warning, just tap the "Warn" button.

While it is not possible to provide a 100% demonstration of how the Medical History Database Application works on this web site, the following will give you an idea of how things work in the actual application. In the images below, the 'Warn' and the 'Back' buttons and the "!M" cell in Figure 2 are active. If you place your cursor over the buttons and left click the mouse, you will jump to the appropriate image.

  1. The black line is the highlighted condition.
  2. Tap the 'Warn' button to view the warnings associated with this condition.
  3. The vertical scroll bars work normally. You can also scroll by using the center up and down buttons.
  4. The upper right 'X' is the 'Close' button.
  5. The '?' button gives copyright and contact info.
  6. The '!' button is for instructions.
  7. The bottom of the PALM screen (Graffiti area and the basic functions buttons) remain active.
  8. You can enter the number of the condition OR the first letters of the condition (e.g.. 7 OR 'irr' for irregular heart beat) into the dotted underline Text Box just in front of "Enter Name or #" Label to jump to the condition.
